Privacy Policy
This privacy policy governs your use of the Super Care Inc. (SCI) product and service which includes the iBreatheWell application for mobile devices created by SCI. The iBreatheWell application attempts to educate, and assist patients in tracking their health progress by tracking their vitals, entering prescribed medication, setting reminders, and submitting surveys. Super Care, Inc. also does business as SuperCare Health and provides direct patient services.
User Provided Information
The iBreatheWell application obtains the information you provide when you download and register the application. Registration is optional, however please keep in mind that you may not be able to use some of the features offered by the application unless you register with us. When you register with us and use the application, you generally provide, your email address if you are not a SCI patient. For those that are SCI patients, your information will be linked with the email that you provided to the SCI representative. The personal data we receive about you (which we refer to collectively as your personal information) includes registration, feedback information, data devices information, use information and derived information:
Registration information is information you are required to provide in order to register with us when you register with SCI in connection with your use of a SCI product or service, or when you submit information to SCI to request services from SCI. This information may include your name, address, telephone number, email and other contact information; information regarding payment including your health insurance provider, if any, as well as the serial number for your user device.
Feedback Information is information you submit to us in connection with your use of SCI products and services, whether through our website, through our data services, or otherwise, regarding SCI products and Services, or other matters relating to us and our business, including the metadata relating to that information.
Data Services Information is information we receive and transmit through our products and services, including: Information associated with each reading, including date, time, and device identifier. Any of the data generated or used by your SCI products to produce other data services information or otherwise available to us through your SCI Products, such as data used for technical support; Information provided by you or about you by another person you have designated to receive any of your personal information through functionality of a SCI product or service (we call each such person a designated recipient).
Use Information includes: Information we receive from the computer, mobile phone or other device you use in connection with SCI products and services, and information we receive from those products and services regarding your use, which may include your IP address and other information regarding your computer, your internet service, the browser you use, and your activities while using SCI Products and Services, such as how often you open software apps, your settings and other activity regarding your use of the components of SCI products and services; Information we receive from you in connection with our request for comments or feedback on third parties;
Derived Information is information that we create by combining and/or analyzing some or all of your personal information. If any of the personal information described above does not reveal your specific identity or relate directly to an individual, we may use such other information for any purpose, except where we are required to do otherwise under applicable law. If we are required to treat other information as personal information under applicable law, then we may use it for all the purposes for which we use and disclose personal information. Your Rights and Responsibilities
You may update your registration and ordering information at any time by logging into your account, and you agree to keep your registration information current at all times while your account is active. You can also terminate your use of a software app or a data service that requires a software app by removing the software app from your computer, phone or other device on which it is installed. You may also review, correct, update, suppress, or delete your personal information or withdraw your consent previously provided to us by contacting us at COPDhelp@supercare.com. In your request, please make clear what personal information you would like to have changed, whether you would like to have your personal information suppressed from our database, or otherwise let us know what limitations you would like to put on our use of your personal information. For your protection, we may only implement requests with respect to the personal information associated with the particular email address that you use to send us your request, and we may need to verify your identity before implementing your request. We will try to comply with your request as soon as reasonably practicable. Please note that we may need to retain certain information for recordkeeping purposes and/or to complete any transactions that you began prior to requesting such change or deletion. There may also be residual information that will remain within our databases and other records, which will not be removed.
If you submit any personal information relating to other people to us or to our service providers, you represent that you have the authority to do so and to permit us to use the information in accordance with this Privacy Policy.

If you are a resident of the United States, below is a description of how we use cookies and other similar technologies in connection with our Products and Services. Children’s Online Privacy Protection: SCI does not permit any person under 18 to subscribe directly to SCI services or software apps or to directly purchase SCI products. A parent or legal guardian may subscribe for a user who is under 18, but no person under 18 is permitted to use SCI products or services without the express agreement of a parent or legal guardian to the terms of the agreement which require, among other things, that the parent or legal guardian is responsible for designating designated recipients, connecting the user device to SCI services or software apps, interacting in all ways with SCI services and software apps, and ensuring that all communications with us come from the parent or legal guardian and not from the person under 18.
Adults with guardians, conservators or other legal supervision: SCI does not permit any person to subscribe directly to SCI services, or software apps or to directly purchase SCI products if that person lacks the legal competence to enter into a contract and be bound to the terms of the agreement. A legal guardian, conservator or other person with the legal right to do so may subscribe for a user who lacks the legal competence to enter into a contract and be bound to the terms of the agreement, but no such user is permitted to use SCI services or software apps without the express agreement of a legal guardian, conservator or other person with the legal right to provide such agreement to the terms of the agreement, which require, among other things, that such legal guardian or other person is responsible for designating recipients, connecting the user device to SCI services and software apps, interacting in all ways with SCI services and software apps, and ensuring that all communications with us come from the such legal guardian or other person and not from the user who lacks legal competence.
Phishing: We do not and will not, at any time, request registration and ordering information in a non-secure or unsolicited email or telephone communication. Identity theft and the practice currently known as "phishing" are of great concern to us. Safeguarding information to help protect you from identity theft is a priority for us. Do Not Track Disclosure: In accordance with the Do-Not-Track amendments to the California Online Privacy Protection Act, we inform you that we do not currently respond to “do not track” signals or similar messages from your browser.
Retention: We will retain your personal information for the period necessary to fulfill the purposes outlined in this Privacy Policy unless a different retention period is required or permitted by law.
Terminating Your Account
You may terminate your account by contacting SCI at COPDhelp@supercare.com. iBreatheWell users may terminate their usage by deleting the app. When you terminate your account, we may retain your personal information, and we may continue to use your personal information, but only as authorized in this Privacy Policy and by applicable law. You may contact us at the email or mailing address (8345 Firestone Blvd, Suite 210 Downey, CA 90241) to request that we delete your personal information, and we will seek to remove your information from our active systems expeditiously.
Security
We are concerned about safeguarding the confidentiality of your information. We provide physical, electronic, and procedural safeguards to protect information we process and maintain. For example, we limit access to this information to authorized employees and contractors who need to know that information in order to operate, develop or improve our application. Please be aware that, although we endeavor to provide reasonable security for information we process and maintain, no security system can prevent all potential security breaches. SCI cannot guarantee security of your information that may be accessed by other applications on your computer, phone or other device.
Changes
This Privacy Policy may be updated from time to time for any reason. We will notify you of any changes to our Privacy Policy by posting the new Privacy Policy here. By using our web-services and applications, you are consenting to our processing of your information as set forth in this Privacy Policy now and as amended by us. Processing, means using cookies on a computer/hand held device or using or touching information in any way, including, but not limited to, collecting, storing, deleting, using, combining and disclosing information, all of which activities will take place in the United States. If you reside outside the United States your information will be transferred, processed and stored there under United States privacy standards.
